Consent and Intake Form

This consent must be given for all family members before any sessions begin (this includes consent for all individuals from the parent/career). To confirm your sessions please sign this consent form within 24 hours of your triage call.

You are accountable to respectfully show up on time for each session to learn some wellbeing tools & techniques with a holistic approach that will be coached / mentored via the N.O.W's team.

As parents/carers you are also responsible to ensure any children are present on time for each session, passing information onto the school if the sessions are being facilitated within school hours.

If for any reason a session cannot be attended on the time / date booked, a 24hrs notification prior to the session is required (or this will be deemed as a missed session 'no show'). Please notify the N.O.W's team by directly messaging via your dashboard, or emailing enquiries@nowsthetimeforchange.com or by freephone 0345 366 9755, where we aim to reschedule sessions if possible.

If a session is missed and N.O.W's has been notified, the week will be deducted from the overall 6 or 12 week programme with the week missed merged with the following week.

Once the programme is booked, session times can be changed only once, subject to coach availability. If rescheduling your sessions isn't possible, you will be advised by H/O to contact your referrer, where they may refer you back into our services to restart the programme at a later date .

If any 1 session is missed without any notification, it will be deemed as a 'no show', and all further sessions will immediately be cancelled. You will need to contact your original referrer or the N.O.W's team to discuss a re-referral back into N.O.W's.
I agree for N.O.W's to hold my personal information and to share with my referrer & organisations within NHS / NELFT.

Any safeguarding concerns will be discussed immediately with NOWs Safeguarding officer with any further investigation in accordance with the government guidelines.

I agree to my email address to be on the N.O.W's mailing list to be updated on further supporting services / online courses.
I agree for the N.O.W.S. Admin Team to communicate with either myself or my child's school (where sessions are taking place within an educational setting) regarding their programme.

I understand that I am responsible for providing the school with the information and letter supplied by N.O.W.S. within one week of receipt. This will enable N.O.W.S. to make contact with the school to obtain safeguarding contact details, including the Designated Safeguarding Lead and relevant pastoral staff, and to provide the appropriate point of contact with direct links and information required for my child to access their sessions within the educational environment.

School contact will only be made after the parent/carer has shared the information provided by N.O.W.S. with the school.

The N.O.W.S. Admin Team will remind either myself or the school of my child's sessions approximately one week prior to the programme start date.

If my child is attending sessions within school, N.O.W.S. will only contact the school if the child or young person is unable to attend a scheduled session, if there are technical issues preventing access to the session, or if an immediate safeguarding concern arises that requires communication with the school's safeguarding team in line with safeguarding procedures.
I understand if any safeguarding concerns arise during a session, the appropriate action will be followed through the N.O.W.S designated safeguarding lead (DSL).
Do you consent to NOWS collecting and using your assessment and outcome data for the purposes of research, analysis, and the publication of findings in relevant research papers. You acknowledge and agree that the data may be shared with governing bodies or other relevant parties as required by law or for the purposes of regulatory compliance. You understand that your participation is voluntary, and that all data collected will be handled in accordance with applicable privacy and confidentiality regulations.
